Reminder: Disclose If You’re Related to an Agency Employee, Board Member or Anyone Within Your District Assignment

As part of Heartland AEA’s board policy, all Heartland AEA staff are asked to certify that they are not currently related to any Heartland AEA employee or board member or anyone within their district assignment. If there is no such relationship, there is nothing you need to do. If such a relationship exists, please disclose the relationship to Human Resources using this form. Only relationships within your immediate family (spouse/partner, child, sibling, step-child, in-law) need to be disclosed.
