Affordable Care Act (ACA) Reporting – Form 1095-C Mailed to Your Home

The ACA requires Heartland AEA to provide Form 1095-C, Employer Provided Health Insurance Offer and Coverage, to all employees who were enrolled in our health insurance plan at any time in 2016. This form provides information about the coverage offered to you, your spouse and dependents, if applicable, and the months of coverage.

Your 2016 Form 1095-C has been mailed to your home address. You can also view your Form 1095-C through the Employee Online section of your Employee Dashboard. Once in Employee Online, go to the left-hand navigation and click on W2/ACA 1095C Info then on 1095C 2016. This will open a PDF copy of your 1095-C.

Des Moines Schools to Host 2nd Annual Summit on School Climate, Aug. 8-9, 2017

Des Moines Public Schools is hosting its second annual Summit on School Climate and Culture, Aug. 7-8, 2017 at the Iowa Events Center in Des Moines. This event is designed to improve school climate and culture through professional development, skill building and application of the best practices and research. Well-known national experts including Sir Ken Robinson, Chris Emdin, Principal Kafele and Anthony Muhammad will be featured speakers. The summit will have a series of workshops, speeches and breakout sessions focusing on best practices in improving student behavior, MTSS, cultural proficiency, social and emotional health, stakeholder engagement and more. Update on Those Having Issues With the Five to Thrive Challenge

We have been told that this issue has been resolved. If you have multiple active coverages, you will still need to go to the drop down menu and select the Wellmark ID that corresponds to our Wellness program. You should only need to select “GO” once, and after the page refreshes, the correct Wellness Center widget should present. Thank you for your patience while we resolved this!

The Pleasantville Schools has a new web address at www.pvillecsd.org. District emails have also changed to @pvillecsd.org. The old addresses still work as well.

Family & Educator Partnership to Host March Transition Fair in Ames

Heartland AEA’s Family & Educator Partnership is hosting a transition fair on March 7 from 5 to 7 p.m. at the Ames regional office. The fair is an opportunity to link families of students with special needs to community and agency resources. Parents, students ages 14+, educators and AEA staff are invited to attend the fair to gather information that will be of assistance for students with special needs who will be transitioning from high school to post-secondary pursuits.
