Consistency to be Topic of Coming Focus Group Sessions

As part of the on-going attention to building a high performance culture at Heartland AEA, the Agency Planning Council (APC) has asked the Hanover Research Group to conduct structured focus groups with agency staff. The focus group feedback will assist the Agency to refine its work on agency culture, especially in the area of Consistency, as defined by the Denison Culture Survey.

The Agency’s department and regional planning councils nominated staff members to participate in the focus group sessions, which will be held on Jan. 28 and Jan. 30 in the Johnston Regional Education Center. A representative from the Hanover group will facilitate the 1 ½ hour sessions. The feedback provided will be summarized into an agency report; individual feedback will not be identified.

We look forward to these coming sessions and the opportunity to take the next step in our on-going improvement process.

W-2s Mailed Last Week; See Below for More Information

Your 2014 Form W-2 has been mailed to your home address. A help document of tips to better understand your W-2 can be found on the Intranet > Business Office > Payroll Forms & Information. This help document provides an explanation of certain boxes on your W-2.

Want to view your 2014 Form W-2 online? You can! Login to your Employee Dashboard and select the Employee Online navigation tab at the top of the page. Once in Employee Online, go to the left-hand navigation and click on “W2 Info” then on the “W2 2014” link. This will open a PDF copy of your W-2 that you can print.

The Patient Protection & Affordable Care Act of 2010 (also known as the Affordable Care Act or Obamacare) requires employers to report on Form W-2 the cost of employer-sponsored group health insurance. Staff members who participated in the Agency’s health insurance plan at any time in calendar year 2014 will have an amount reported in box 12, letter code DD of their W-2. The required amount to be reported is the sum of employer and employee premium contributions. Please note that this is informational only and the amount reported is not taxable.

We have added a date screening date for Johnston on Jan. 20. Screenings will be held in Rooms 139, 104 and 108. If we do not have 10 people signed up for these dates, we will have to cancel them. The Johnston screenings on Jan. 21 will be in the Johnston Regional Education Center (Rooms 104, 108 and 113).

If you do not end up making an appointment, you can try to walk-in to one of the screening locations, but it is NOT guaranteed that you will receive a spot, but it is possible due to cancellations. Remember, you will need to have fasted for 8 hours prior to the screening.

And remember, you can still make your own appointment at Hy-Vee as long as there is a dietician on staff at the location you choose to visit. Be sure to call ahead to your local Hy-Vees before you schedule your screening, as walk-ins CANNOT be guaranteed.
