This message was emailed to staff on Jan. 8 and is being repeated here as a reminder to complete the survey. 

This survey is for full-time Heartland AEA staff who have health insurance through the Agency. If this does not apply to you, please disregard. 

Please complete the survey below to gauge your feedback about insurance and other benefits offered through the Agency. The results will be used to guide the Agency Insurance Committee's decision making. Your responses are anonymous and will be viewed by the Insurance Committee.
